  • Next, a hospice care professional will visit the patient in their home and assess their level of need. 
  • Based on this assessment, Crossroads will provide medical supplies and equipment. 
  • Finally, hospice care is administered in the patient’s Delaware County home. These services are not a replacement for care from the patient’s primary care physician, but instead go hand-in-hand with other services the patient may be receiving.
